Program

Concierge Services with Enhancements

Your PCP’s goal is to have a long-term care relationship with you. This will allow Michelle, as your PCP , to be better able to discern changes in your health. Your PCP will also serve as an “entry point” to health care if your diagnosis is not clear and will help you navigate the sometimes-complex health care system by coordinating with other specialists, making referrals, and staying on top of chronic diseases.

Your PCP will treat and monitor a wide range of common health issues, such as diabetes, high blood pressure anxiety and depression. You PCP will recommend necessary immunizations, order bloodwork and necessary urine tests, other diagnostic testing, perform removal of impacted cerumen, if necessary, treat  minor infections, and send referrals for VNA (Visiting Nurse Association) services and for PT/OT/ST if necessary.

You will receive annual wellness/physicals visits. You will also receive other health screenings to uncover certain conditions early, so that you have more time to consider and address your health goals. Your Primary Care Provider will collaborate with you to develop a care plan that you can follow, with the goal of avoiding a situation where you end up in the hospital or need more intensive care.

Enhancements:

The enhancements that you will receive by utilizing Mindful HEART, LLC’s  concierge medicine practice is listed below.

  1. Individualized Care Plan. Your care plan will be individualized for you and targeted to address your specific needs. This will entail setting (SMART) specific, measurable, attainable, relevant, and time-based goals to enhance your well-being.
     

  2. Panel Limits. No more than fifteen (15) patients will be admitted to the Practice. This number will be reassessed on a yearly basis and is subject to change.
     

  3. Direct Access. Mindful HEART, LLC will provide you with detailed instructions on how to contact your Primary Care Provider by phone, text, and email (collectively, the “Communication Enhancements”) for scheduling or changing appointments, to request prescription refills, or to discuss medical questions or concerns.
     

  4. Visit Locations. The Practice will provide the Professional Services at your home.
     

  5. Same Day/Next Day Appointments for Acute Issues. The Practice will arrange for you to be able to use the Communication Enhancements to schedule non-emergent in-home acute visits within one (1) to two (2) business days of your request.

6. Extended Patient Appointments. The Practice will arrange for you to be able to have appointments with your Primary Care Provider for routine visits of such length as you reasonably need to review and discuss your concerns, regardless of medical necessity.
 

7. Care Coordination. The Practice will arrange for your Primary Care Provider, working with hospitalists and other practitioners, to be involved in coordinating or overseeing your medical care outside the office to the extent reasonably feasible, including in the hospital, skilled nursing, rehabilitation, or home setting. The Practice will also arrange for your Primary Care Provider to conduct a pre-visit verbal or email consultation with those specialists to whom you are referred, to conduct appropriate follow-up with respect to your specialist visits, and to review the specialists’ recommendations with you by phone, email, or in-office discussion, at your option.
 

8. Mind and Body Medicine Program. The Practice will incorporate the Harvard Medical/Massachusetts General Hospital Mind and Body Medicine program into each of your visits through tools that you will learn by taking the required Benson Henry Institute SMART (Stress Management and Resiliency Training) program. This will:

  1. Empower you to take control of your own health:

  2. Allow the focus of your visits to be on wellness, not just illness;

  3. Practice and reinforce techniques that elicit a relaxation response;

  4. Aid your knowledge of the nexus between stress and physical/emotional problems;

  5. Emphasize the role of positive thoughts and beliefs in support of mind and body healing;

  6. Encourage you to successfully incorporate mind and body principles and practices, including socialization, into your personal health; and

  7. Strengthen your understanding of the importance of healthy eating and physical activity to your overall health and well-being. This will involve goal setting and providing the necessary resources to help you attain these goals.

9. In home lab draws. Exam One, A Quest Diagnostic Company and other diagnostic testing  (x-rays, ultrasounds, EKGs) through Trident Care for an additional cost.

Note: Membership Fees paid by you are solely for the Enhancements listed on this Schedule that are furnished, or arranged to be furnished, by the Practice. The Membership Fees do not pay for any Professional Services, or any items or services covered by your health plan. If any Enhancement is construed to be covered in part by your health plan, the Membership Fee pays only for that portion of the Enhancement that is not so covered.

Membership Fees

***In addition to the following, the Practice will provide the Professional Services listed, which are not included in the Membership Fee. Professional Services will be submitted for reimbursement to your third-party payor and/or billed to you separately.

Year 1 Membership Fee: $3,000

  • Includes 8-week Harvard Medical Benson Henry Institute SMART program with course materials. ***This is required to be admitted into the practice.

  • ​Includes Enhancements as seen above

Year 2 Membership Fee: $2,500

I have never been a very "mindful" person. When Michelle asked me to join her 8-week Stress Management and Resiliency Training Class I was reluctant... But then I thought, maybe this will be good for me. I can honestly say that this class was extremely beneficial to me in many ways. I have learned that it is so important to live in the moment and practice self care. Throughout the 8-weeks, Michelle provided us with tools, meditation techniques and relaxation strategies to help us during stressful situations. I want to be a healthy and a positive role model for my daughter. It is important to me to model the practices that I have learned for her so that she can live her best life too. Thank you Michelle for your expertise, guidance and the amazing way that you have taught me life changing practices for handling stress. I am truly grateful.
